For compliance purposes we also record the date and time of submission, the page you submitted from, your IP address, your browser user agent and, where given, your SMS consent and the exact consent language shown to you.

Mobile information

No mobile information or SMS consent will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are not shared with any third party.

Mobile phone numbers collected for SMS are not shared or sold to third parties or affiliates for marketing purposes.

We use SMS only to communicate with people who have opted in: appointment confirmations, scheduling updates, reminders, and follow-ups related to your inquiry or service request. We do not send unsolicited marketing messages.

Retention and security

Submissions are stored in an access-controlled database and retained for as long as required to service your relationship and satisfy recordkeeping obligations.
